Kicked out?

I just saw a player get tossed from a game 'for violating the rules.' Questions:

A) what rules can be violated? I would have the thought the architecture of the game is the architecture of the game.
B) how would that person then be kicked? Or even detected?

Usually it means a violation of Rule #1: No multiple accounts.

Sometimes it can mean other rules, but 99% of the time when you see that message, it means the person got caught running multiple accounts.
