My first tournament I hosted was concluded today. I posted in the thread the winner, and I also changed the tournament title. Is there anything else I have to do to officially conclude a tournament?

I'm not your most reliable source, but I can tell you my experience.
I hosted a tournament once and all I did were exactly the things you just named. So no, I think you're all set. You'll get your medal once the Tourney Mods have seen your tourney thread.

You are all set, the TD's check the forum daily and will issue medals and move the thread the next time they check
