Ev, as far as I know, they all have the same weight as far as the score goes - each team game counts as 1 game, as does each speed game. However, if your team wins a quads game, (I believe) each of you gets credit for whac-ing all 4 mods you played against.
Each player only gets one medal, but in the explanation on your profile, it will say how many mods you whac'd.

If you win the game, then yes. But if not, then no, unfortunately.
The player who wins the game gets credit for the mod whac, even if he/she did not eliminate the mod.
Partly because, when you consider that there are almost 650 games in this year's event, going through all of them to determine who actually eliminated which mod would take too much time and effort - especially for team games, where there are multiple mods to be whac'd.
