The books don't clearly explain exactly how he went about changing his name from Tom Riddle to Voldemort... there could have been an intermediate phase when he was still officially called "Tom" but he was already experimenting with "Voldemort" as a nickname.

He was known as voldemort by his ally's and inner circle as early as 16.

Book 2 and 6 clearly explain this.

There are times that he makes his turns and he sometimes do not. The last time he went online was on the 21st of October so he must have some personal stuff to deal with. For now, I am going to clear this and also note the report for reference.
