Bring your own fork. Go buy one. Share the fork. Split it in two with your bare hands. It's easier than getting one from them.

He could just ask for an extra plate but it would probably cost $60

I think the fork (as well as being an actual fork) is a metaphor. I don't think taking your own fork would stop them charging you. DUnno. Never going in there. (It also means that there is going to be not a single dish that's less tha $18 in there...)

Sounds like you should expect them to be in the $40 range.
